1. A statement, minimum of 150 words, of professional goals and the
intent for use of the award, which must be pertinent to Occupational Health
Nursing.
2. Two letters of recommendation with at least one of the letters coming
from an occupational health nurse peer.
3. List Occupational Health Nursing Organization Experience (i.e.
Offices held at state or local level)
4. List formal educational background
5. Note if currently certified, or is seeking certification as an
occupational health nurse
6. The completed application will be submitted to the Chairperson of the
Educational Grant Committee by July 1, 200_.
7. Applicants will be reviewed and a selection made by the TAOHN
Educational Grant Committee. The committee will consist of four members and
a chair. The winner automatically becomes the chair for the next consecutive
year. The president and board of directors will appoint the members of the
committee.
8. Members may apply more than once, but may only receive the grant one
time.
9. The chairperson will purchase a plaque and be reimbursed by thee State
Treasurer.
10. The grant will be awarded at the TAOHN Annual Conference.
11. The State Treasurer will issue a check to the member selected. The
grant must be used within one year of being awarded and receipts sent to the
State Treasurer.
12. The winner’s immediate supervisor will receive a letter notifying
them of the member’s award.
